Nephew's Symptoms

Both my brother and my sister-in-law have not taken my nephew to a physician, pediatricain or any type of doctor at all. He does not speak at all, nor babble, and does not know who I am or for that matter his cousins, my three kids.
He becomes absorbed with the same video tape at the latest count 300 times, and he constantly lines up bottles and cars; not to mention the fascination he has with turning on and off switches. My sister -in -law wants to believe that he is so analytical on the left hemisphere of his brain, that it is slowing down the speech area of his same hemisphere.
